Which path fits me?

Original Medicare + Medigap, or Medicare Advantage?

Original Medicare (A + B) + Medigap + Part DMedicare Advantage (Part C)
DoctorsAny doctor or hospital in the U.S. that takes MedicareUsually a network; going outside may cost more or not be covered
Monthly costPart B premium + Medigap premium + Part D premiumPart B premium + plan premium (some plans $0)
Costs when you get careLow and predictable with MedigapCopays per visit; yearly out-of-pocket limit
Approval before careRarely neededOften needed for some services
Extras (dental, vision, hearing)Not usually includedOften included
Switching laterMedigap may ask health questions after your first windowCan change each year Oct 15–Dec 7

Plain rule of thumb: If you want freedom to see any doctor and predictable bills, and can afford the Medigap premium, Original Medicare + Medigap + Part D fits. If you want a low monthly premium and extras, and your doctors are in the network, Medicare Advantage fits. Buy Medigap in your first 6-month window if there's any chance you'll want it.

Step-by-step for open enrollment (Oct 15 – Dec 7, 2026)

  1. Read your Annual Notice of Change (mailed by your plan in September). It lists what changes on January 1.
  2. Make a list of your drugs (name, dose, how often), your doctors, your hospitals and your pharmacy.
  3. Go to the Medicare.gov plan finder (or call 1-800-MEDICARE, 1-800-633-4227). Enter your drugs and pharmacy and compare yearly total cost, not just the premium.
  4. Check networks: call your doctors and ask, 'Are you in this plan's network for 2027?'
  5. If money is tight, apply for Extra Help at ssa.gov and ask your state Medicaid office (TennCare in Tennessee) about Medicare Savings Programs.
  6. Get free help from your SHIP counselor (Tennessee: 1-877-801-0044). They do not sell insurance.
  7. Enroll by December 7 on Medicare.gov, by phone, or with the plan. Joining a new plan automatically ends your old Advantage or drug plan.
  8. Keep proof: write down your confirmation number. Watch for your new card and welcome letter.

FAQ

When is Medicare open enrollment for 2027 coverage?

October 15 to December 7, 2026. Changes you make start January 1, 2027. If you're in a Medicare Advantage plan, you also get one change from January 1 to March 31.

What is the Medicare Part B premium?

The standard Part B premium is $202.90 a month in 2026, with a $283 yearly deductible (CMS). CMS usually announces the 2027 amount in mid-November.

What are the 2027 Part D limits?

Medicare.gov says no drug plan may have a deductible over $700 in 2027, and out-of-pocket costs for covered drugs are capped at $2,400 in 2027 (up from $615 and $2,100 in 2026).

Who pays IRMAA?

For 2026, people whose 2024 modified adjusted gross income was above $109,000 (single) or $218,000 (joint) pay more for Part B and Part D. Social Security decides and you can ask for a review if your income dropped.
